A red, painful throat can have several causes. Knowing the symptom pattern helps, but a throat swab may be needed to confirm strep.
Quick take: Viruses cause most sore throats. The Centers for Disease Control and Prevention estimates that strep causes about 1 in 10 sore throats in adults and 3 in 10 in children. Testing helps avoid unnecessary antibiotics and supports the right treatment when strep is confirmed.
Some symptoms make strep more likely.
Strep throat is a bacterial infection of the throat and tonsils. Symptoms may include a sore throat that starts quickly, pain when swallowing, fever, red and swollen tonsils, swollen lymph nodes at the front of the neck, tiny red spots on the roof of the mouth, or white patches on the tonsils. Children may also experience headache, stomach pain, nausea, vomiting, or a rash.
Other symptoms point more toward a virus.
A cough, runny nose, hoarseness, or pink eye makes a viral cause more likely. Still, appearance alone cannot confirm strep. If the history and exam suggest group A strep, a healthcare professional may perform a rapid strep test. Depending on age, symptoms, and the rapid-test result, a throat culture may also be needed.
Why the correct diagnosis matters.
Antibiotics do not treat viral sore throats. When strep is confirmed, prescribed antibiotics can shorten illness, reduce symptoms and spread, and help prevent complications. Take the medication exactly as prescribed; do not save doses for later or share antibiotics with someone else.
Comfort measures while you recover.
- Sip fluids often and choose warm or cool drinks that feel soothing.
- Rest and avoid smoke or other throat irritants.
- Use a clean humidifier if dry air worsens discomfort.
- Ask a clinician or pharmacist which over-the-counter pain or fever medicine is safe for you or your child.
